Fees for Service: $350-$450 per unit
Cost for Travel: $500 Travel + Hotel (invoiced within 48 hours after completion of appointment and payment by credit card on file)
Deposit: $500 (at the time of booking)
Payment: Payment is made at the completion of appointment by credit card on file. There are no acceptions.
Cancellations/Rebooking Fee (within 21 days of appointment date): $500 Travel + Hotel
Cancellations/Rebooking Fee (prior to 22 days from appointment date): $0
Materials & Equipment: Must be ordered and readied at the time of appointment
Requirements & Recommendations: Digital Smile Redprint (DSR - 3D Mockup) is highly recommended for the predictable results and satisfaction.
Remakes/Redos: Same Day Smile crowns, veneers and or implant restorations do not carry a warranty. All restorations are final at the completion of the appointment. There are no acceptions. Should a defect due to craftsmanship occur, after careful investigation, Master's Arch may offer a one-time remake or repair for 15% off the original cost of each unit. There are no warranties at any point in the remake or repair process. Please remember, the great benefits we have during this "golden time" appointment is that we have both the master ceramist and doctor in the same place working towards our patient's happy smile.
