In efforts to support our providers with ways to create more effective consultations on major cases, we came up with a comprehensive process we call, A+ Case Accept.

  • How do I download paperwork online?
    You may download paperwork online with a GUEST CODE from your Account Manager. Please contact for your GUEST CODE.
  • Where can I get a shipping label to send a case?
    Shipping labels can be shipped to you via FedEx along with our case kit or we can email the shipping label to you within minutes. Ways to Contact Us: EMAIL - CALL - 480-622-2222
  • Do you accept Digital Impressions?
    Yes! We accept all .STL formatted digital impressions and CEREC files via Sirona-Connect. For .STL files, you can drop it in our shared Dropbox or email it directly to For CEREC files, once you "favorite" our laboratory by looking up zip code: 85249, Master's Arch, you can Sirona-Connect it over. Once we have accepted your digital impression, your Account Manager will email you with a confirmation of case entered into our system for processing.
