Service Description

When booking a Surgery & Conversion, please describe the case information along with patient's initials to ensure we are all on the same page for D-Day. IMPORTANT: THIS IS A BOOKING REQUEST AND IS NOT CONFIRMED UNTIL YOU RECEIVE ANOTHER EMAIL FROM MASTER'S ARCH. Please note: There is a $650 non-refundable travel arrangement deposit plus other travel fees, should it exceed $650. This deposit is non-refundable with no exceptions. Upon cancellation and or rescheduling, this deposit will cover lost fees. Please consult with our team for more information. After the first two(2) cases of full arch surgery and conversion services with Master's Arch, the cost for this service is $1,500 + travel fees going forward.
